Preservation Rhinoplasty is a rhinoplasty technique that has recently become quite popular among facial plastic surgery specialists worldwide.
Although this particular method was developed many years ago, by the pioneers of rhinoplasty, it was abandoned due to the ease of open access. Research, however, proved the mistakes and side effects of an aggressive ablative rhinoplasty and thus the Preservation Rhinoplasty technique came back to the fore in a more mature and newer version.
This operation is unique, as on the one hand it combines free breathing with the excellent aesthetic result, and on the other hand it does not alter the surface of the nose, we do not have the usual side effects from surface intervention on the bones of the nose.
The most famous facial plastic surgeons worldwide are now applying the new form of preservation and at MITERA the technique is now in the daily practice of the Department of Functional and Reconstructive Rhinoplasty, which is under the direction of Ms. Kypraiou
The design of the Preservation Rhinoplasty method
Preservation Rhinoplasty is characterized by three important steps:
1. Elevation of the soft tissues of the nose to a certain level, so that the skin ligaments are preserved, something that does not happen in classic rhinoplasty.
2. Preservation of the bony ridge, without creating an open roof (as in all previous types of rhinoplasty).
3. Preservation of the winged cartilages of the nose, while the desired shape of these is achieved with sutures and not by removing part of the cartilage.
When dealing with nose deformities with a hump (hump) on the dorsum, the skin at the dorsum will not be detached, as is often done in conventional rhinoplasty, to cause possible visible protrusions and deformities on the dorsum of the nose. Instead, the bones at the base of the nose, low, where the nasal bones begin, will be sculpted, so that the hump will go down and straighten to the desired level, while the surface of the nose will remain intact.
To whom is the Preservation Rhinoplasty technique applied?
The Preservation Rhinoplasty operation treats all types of nose with high success rates, while it has impressive results on a crooked nose.
Ideal. Sure. candidates for the preservation method are young patients who have not undergone surgery in the past therefore, the subcutaneous tissues and ligaments are intact, the bones have not suffered violent fractures and the cartilages of the nose are also intact.
In addition, it is indicated in cases with thin skin, which is particularly sensitive and easily erases protrusions and irregularities on its surface. At the same time, cases with a crooked nose, congenital asymmetries, asymmetric nasal bones as well as trauma that caused a deviation in the nose, are the best indication for the new preservation rhinoplasty.
What is hybrid structural – preservation rhinoplasty?
In cases that have already undergone one or more nasal septum operations with or without rhinoplasty, without success, the new structural-preservation hybrid operation is proposed.
Cartilage grafts are obtained from the same patient, with donor sites being the nasal septum, otic cartilage, and costal cartilage. The grafts will restore the structure of the nose, in terms of shape and aesthetic image to a very satisfactory level.
In addition, the support on the side walls of the nose as well as the stylet and the top will ensure free breathing, correction of snoring and problems of shortness of breath, fatigue, etc.
In addition, nose skin care is an integral part of modern rhinoplasty. Ligaments that are under the skin are identified and put back in place (smash rhinoplasty), while there is a possibility of additional fat or fascia implantation.
The aesthetic result in this way is superior, the nose has a particular naturalness and plasticity, while the results remain unchanged over time.
How is the operation performed and what are the results?
The operation is performed under general anesthesia, while its duration depends on the patient.
• The results of a well-executed new preservation rhinoplasty are safer and long-lasting.
• Swelling is significantly less than a conventional rhinoplasty, so the results are immediately visible from the very first week.
• The aesthetic lines of the nose are kept uniform and no interference with the nose is betrayed. The preservation of the flaring cartilages of the nose, as well as the preservation of the ligaments on them, have dramatically changed nasal surgery on other levels.
in conclusion
While the techniques are constantly evolving, the philosophy now remains the same: tissues must be preserved while being modified.
All patients can benefit from this new method, especially those who will be operated on for the first time, and therefore the technique will be able to be fully applied by surgeons specialized in rhinoplasty.
