Urogynecology is a rapidly evolving branch of gynecology. This is no coincidence, as about half of the women will at some point need appropriate specialized help in progression and/or urinary incontinence.

But what exactly are these problems?

Both uterine prolapse, bladder and rectum prolapse (cystocyle and orthochil), and urinary incontinence, are caused by loosening of the pelvic floor. Over time there is a relaxation of the uterine support mechanisms, the bladder and the rectum. As a result, these organs gradually fall into the vagina. At the same time, there is often a relaxation of the bladder muscle clamp, which leads to the appearance of urinary incontinence.

Why do they appear so often?

The relaxation of the pelvic floor with the consequences we have described is an expected development, which occurs over time. Chronic physical fatigue, regular weight lifting, manual labor, standing, pregnancies and childbirth, loss of postmenopaus estrogen are key factors that will lead to these problems.

How does a woman feel with such symptoms?

Both the prolapse and the urinary incontinence affect the woman’s daily life to a large extent. Weight/foreign body feeling in the vagina, mass perceived in palpation, abdominal pain or waist, constipation, difficulties in urination, frequent and urinary tract infections are the most common symptoms. In cases with advanced prolapse, we also have a renal function. But these situations have a huge impact on mental health as well. Woman reduces her social activities and closes herself. Sex life suffers and often stops, as the woman feels insecure and ashamed. The relationship with the partner and family life are seriously disturbed. Unfortunately, prolapse and incontinence continue to be taboo, so many women do not seek help and suffer for no reason.

To whom should he address?

Today we live in the age of specialization in medicine. The woman should therefore seek the help of a specialist urogyneicologist who has received proper training and has the necessary experience for the proper diagnosis and treatment of these problems.

Is the diagnosis difficult?

No. In almost all cases it is possible in the context of a simple gynecological examination and an ultrasound in the clinic. After identifying the problem, we will suggest the appropriate therapeutic solution, which will ideally meet the needs of this woman.

What is appropriate surgical treatment?

In previous years, treatment was done vaginally or with open surgical incision. In modern urogynikology, the treatment of these conditions is laparoscopic. Laparoscopic methods reduce the likelihood of reappearance of these problems, as well as the risk of complications. Thus, with 3 small incisions, we have the ability to correct uterine prolapse, cystocystick, orthocyte and urinary incontinence in the context of a single laparoscopic surgery and the woman to return home the next day.

What risks are there?

These interventions do not pose particular risks when they are done by a specialized and experienced urogyneicologist. As with any operation, the surgeon’s specialization and experience in this area are the basic prerequisite for its success and safety.

So what advice would you give to a woman with prolapse and/or urinary incontinence?

To talk about her problem. The woman in our time is not allowed not to seek help because she is ashamed of her problem or because she considers it normal or expected for her age.
