The result is described as “exciting”, “amazing” and “unbelievable rejuvenation”. It also comes with the warning that the face “gets really ugly”, “a monster for a week” and “it’s like a big burn.” It is the deep layer phenol peeling, an aggressive procedure that promises to change the skin of the face for a new one.
The technique, however, if done correctly is safe. Experts say that it is enough to follow the correct protocols and that the procedure is carried out by a medical specialist – they also recommend that it be done only on fair skins or with low melanin.
Its main indication is for the treatment of wrinkles, but it is also used to treat melasma and acne marks. The result, however, is not as impactful or predictable as in aging cases.
The patient needs a preparation that takes 15 to 30 days, and has to do a careful postoperative period, taking up to a year and a half for the face to heal completely and show the final result. The advantage is that it very rarely needs to be redone, at most retouched, and even then, after a long time.
“The deepest phenol peeling has an unbelievable rejuvenation and a very long-lasting result. There are reports of the effect having lasted 20 years, as long as the person takes care of that skin with retinoic acid and sunscreen continuously”, says dermatologist Ediléia Bagatin, coordinator of the Department of Dermatological Cosmiatry at SBD (Brazilian Society of Dermatology).
She reinforced that deep phenol peeling can only be done by dermatologists or plastic surgeons. In both cases, the professional must be specialized in the technique.
“It is not a surgical procedure, but it is considered invasive. It causes a great aggression, as if it were a deep chemical burn, which will require almost daily follow-up, at least in the first week. patient”, says the doctor.
The procedure must be performed in a surgical center or monitored outpatient clinic because of anesthesia and arrhythmia peaks that can be caused by phenol in the bloodstream before its metabolization by the body. The application is done in stages to avoid high concentrations, in addition to overloading the heart.
“The patient must always be very well oriented about what is going to happen. The postoperative period is very painful, the aspect that becomes very ugly”, explains the doctor.
In 15 days, the entire skin flakes off and it is already possible to see the result despite the rosy tone. At this stage hygiene and hydration are important, in addition to a lot of sun protection. The ideal is not to leave the house, at least in the first week, according to Bagatin.
Allan Bernacchi Costa, 47, a specialist plastic surgeon at the SBCP (Brazilian Society of Plastic Surgery), says that phenol peeling has been used in medical treatments since the 19th century, but began to be applied for rejuvenation in the 1960s with Baker’s formula. -Gordon, gaining fame in Brazil with the less toxic technique of the doctor José Kacowicz.
“Phenol peeling is a safe procedure, and today it is much less toxic than when it was developed to treat wounds”, he says.
Costa also recalls that extreme reactions of intoxication happen when you don’t look for a qualified professional. He states that the Brazilian skin type, in general, does not allow a deep approach, since the treatment is not indicated for skins with a higher concentration of melanin.
Ricardo Kacowicz, 41 years old, facial plastic surgeon and son of José Kacowicz, has been working with phenol peeling for 15 years. The surgeon says that the method is very safe compared to other interventions, since there are no records of deaths from the use of the technique. Possible poisoning of the liver, kidneys and heart are the result of procedures performed improperly, he said.
“Any person who is healthy or who has treatable comorbidities, such as controlled diabetes, controlled high blood pressure, controlled thyroid, can do it. From a skin point of view, lighter people, with wrinkles. cases of darker people, but I can only identify them personally and they are exceptions”, says Kacowicz.
The metabolism of phenol in the body, according to the doctor, happens like any other drug, such as dipyrone or paracetamol. However, when there is a high concentration of phenol in the bloodstream, there may be benign cardiac repercussions.
“All you have to do is identify it on the monitor, stop the technique and wait for the body to metabolize that phenol and then come back and finish the peeling. In addition, if the person has a liver, heart or kidney problem, I will always ask for guidance and release from a specialist in the area. “, highlights the surgeon.
Rosangela Bitencourt Ferreira Ramos, 62, was one of Kacowicz’s patients. She learned about the procedure on social media and in the same week decided to do it. Her goal was to renew her skin by reducing stains and sagging.
“I recommend it to everyone who wants to recover the skin damaged by time, there is no better treatment. You just have to have a good professional and you won’t regret it, it’s very good and it was beyond what I expected”, he says.
Phenol peels are mostly sought after by women, but the number of men has been increasing in recent years (and in both cases, the appearance of aging counts more than age). A treatment with phenol can vary between R$ 15 thousand and R$ 30 thousand, depending on the situation and the desired result.
Former actress Maria Eli da Silva says she doesn’t regret having done it and would do it again. “I was a television and film actress in the 80s. Then I graduated in aesthetics and a few years ago I discovered phenol peeling and fell in love. I am 60 years old and I certainly don’t look like it. to phenol peeling”, says Silva, who underwent the procedure eight years ago.
Priscila Galzo Marafon Moda, 45 years old, dental surgeon and lawyer at Marques e Tasoko in the field of Medical Law, recalls that phenol peeling is characterized as a high-risk technique and must be monitored before, during and after the procedure.
The professional needs to know the origin of the products, if they have been released by Anvisa (National Health Surveillance Agency) and carry out a good anamnesis of the patient’s general health, in addition to making a service provision contract that contains a Free and Informed Consent Term. signed.
The document must bring items such as risks of the procedure, returns, how the intervention will be carried out and care throughout the recovery period.
“If the procedure causes harm to the patient, and if he is not informed about this possibility, and the professional does not know how to reverse or mitigate the damage, he can be prosecuted in the civil, criminal and administrative spheres”, says the lawyer.
