Robotic and endoscopic treatment of prostate diseases

The prostatic fluid it secretes is an important part of the sperm and contributes to the motility of the sperm. It also regulates the alternation of urination and ejaculation, as it participates in the formation of the internal urethral sphincter.

Minimally invasive treatment of Benign Prostate Hyperplasia
In the majority of cases of surgical treatment of benign prostate enlargement, endoscopic techniques are preferred, through the urethra (transurethral access) and without incisions. The most commonly used techniques worldwide are transurethral prostatectomy using bipolar diathermy (TURis), the minimally invasive Rezum technique, and laser prostate nucleation (HoLEP).

“These modern techniques show excellent results in restoring the quality of urination, with a very low probability of complications.

For patients with mild prostatic hyperplasia who wish to maintain their ejaculation or who cannot receive anesthesia, the Rezum technique can offer an important solution. Thermal energy is used in the form of water vapor which is deposited in the prostate tissues and causes cell necrosis. “The results of the treatment usually appear within 2 weeks”, explains Mr. Nikolaos A. The TURis technique, using bipolar energy in saline, is selected for medium or larger prostates with significant success rates and a result that the patient sees immediately, while minimizing complications compared to the techniques used in the past (monopolar energy, open surgery ).

For extremely large prostates (over 80 grams) the laser nucleation of the prostate (HoLEP) is indicated. The technique ensures removal of the entire prostate adenoma without the need for incision and with a low chance of complications.

Robotic Prostatectomy for Prostate Cancer
Prostate adenocarcinoma is the most common cancer in men and the second most common worldwide. However, with early diagnosis and treatment, the rate of prostate cancer deaths remains quite low.

With the introduction of Robotic Surgery, it is being treated with the Da Vinci Xi Robotic System which is the latest in technological advances in surgery.
“During the operation, the prostate gland is removed together with the seminal vesicles and part of the seminal vesicles. The bladder is then sutured back to the urethra. In cases of aggressive cancer in the initial prostate biopsy, the iliac lymph nodes are removed prophylactically.

This technique increases the possibility of faster restoration of the patient’s restraint and erectile function, combined with an excellent and safe oncological result. The patient’s recovery after surgery is much faster than open prostate removal and the pain is reduced to a minimum. “The probability of complications is much lower than the open technique”, concludes Mr. Costakopoulos.
