According to an announcement by the Coast Guard, during loading, the 24-year-old driver did not submit a balance sheet, did not proceed with the timely declaration request for approval to load dangerous goods, to the ship’s master or the maritime agent, nor did he proceed to inform the relevant Port Authority. In addition, it was found that he did not hold a certificate of professional training for drivers of vehicles transporting dangerous goods, as he should have.
